Our Bridging Program is designed to assist seniors in their transition from Acute Care and in-Hospital Rehabilitation Care back to their homes in the Community.
HOW IT WORKS
Seniors currently in acute care at Saanich Peninsula Hospital (SPH) and who are suitable candidates for the Day Long Program following discharge from the SPH may be referred to Mount Newton Centre for physical and mental stimulation, socialization and activation.
Mount Newton Centre staff have a wide range of resources to draw from including a comprehensive Medical Equipment Loan Service. The Centre's RN and staff will assist with assessment in the Centre's home-like atmosphere.
Mount Newton Centre will provide transportation to and from the hopsital for participants of this Program providing they will be eligible for the Centre's Day Long Program following discharge from the hospital.
Former Mount Newton Centre Day Long Program clients, and seniors currently receiving acute care or rehabilitation (i.e. post stroke, joint replacement) at Saanich Peninsula Hospital may attend the program. A referral to the program may be initiated by a physician, social worker, or hospital liaison nurse, but all referrals must be approved by the patient's physician.
